29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    On the north side of the construction site, an employee engaged in installing metal straps from canopy used a pneumatic nail gun and was not wearing eye and face protection exposing him to struck-by hazards and eye injuries.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On the north and south side of the construction site, framing employees were engaged in installing roof bracing  above the service yard and metal straps from an overhead canopy without fall protection and exposed to a fall hazard of approximately 17 feet to the ground.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided.     On the north side of the construction site, the employer did not provide a ladder where the employee could access and egress the buildings overhead canopy.  The employee was exposed to a potential fall hazard.
