At the worksite - On or about August 17, 2016, employees used the top and top step of a eight foot Werner (model: FS108) fiberglass step ladder to access/egress the top plates, thus exposing employees to a fall hazard of approximately 11.4 feet onto concrete flooring.

29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures. a). At the worksite - On or about August 17, 2016, there were numerous electrical extension cords and air hoses that covered the interior passageway (i.e. hallway) of the structure, thus exposing employees to a trip and fall hazard.
