The employees were not protected by a personal fall arrest system.

(a) 415 Woodbridge Dr. Charleston WV: On or about August 19, 2016, and times prior, employees were exposed to a fall of approximately 11 feet 9 inches to the lowest portion of the ground, while conducting roofing work on a 5 in 12 slope roof. Two employees were tied off to the same anchor point which is not rated for more than 5,000 pounds.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    (a) 415 Woodbridge Dr. Charleston WV: On or about August 19, 2016, and times prior, employees were exposed to a fall of approximately 11 feet 9 inches to the lowest portion of the ground, while conducting roofing work on a 5 in 12 slope roof. An employee had too much slack in the rope and would have hit the concrete parking pad while wearing fall protection.
