29 CFR 1926.451(g)(2): The employer did not provide fall protection for employees erecting or dismantling supported scaffolds:      On or about August 20, 2016, an employee was exposed to fall hazards at approximately 12.0 feet where it was feasible to wear fall protection.  The employee while on the roof used the rope in which one end was tied to an anchor point.   The employee did not use a harness and wrapped the other end of the rope around his waist, while erecting the top of the pump jack scaffold.        Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
