29 CFR 1926.501(b)(12): Each employee engaged in the erection of precast concrete members (including, but not limited to the erection of wall panels, columns, beams, and floor and roof "tees") and related operations such as grouting of precast concrete members, who is 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems.    On or about August 15, 2016 at the above addressed jobsite, employees performing erection of precast concrete floors on a residential building were exposed to a exterior fall hazard of approximately 15 feet on the north east side of the building. Employees were not protected from falls, thereby exposing employees to fall hazards.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

The employer was not operating the crane as specified by the manufacturer.    On or about August 15, 2016 at the above addressed jobsite, the owner of the company was operating a Grove RT 880E crane that was not on a firm level surface resulting in displacement of the crane's right front. Employees were exposing to a tip over and crushing hazard.      Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
