29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end.   Location - 14 Leggs Hill Road, Marblehead, Massachusetts; On or about August 17, 2016 employees were installing a new roof working from a ladder jack scaffold platform that extended 5 feet 6 inches over its support.

(a) Location -14 Leg Hill Road, Marblehead, Massachusetts; On or about August 17, 2016 employees working on a roof that was 10 feet 9 inches above the ground were not protected from falling.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) Location -14 Leg Hill Road, Marblehead, Massachusetts; On or about August 17, 2016 the employer failed to train employees who were working on a roof that was 10 feet 9 inches above the ground.
