29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:       (a) On August 19 and times prior, in the repair bay of the facility, employees were exposed to injury in that the fabricated man basket attachment that was used with the Powered industrial Truck for elevated work tasks, did not have written approval from the manufacturer. Determination of affected capacity and safe operation can not be made without written approval from the manufacturer.        Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
