29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about August 17, 2016 employees were not wearing hardhats being exposed to struck by hazards from an excavator used onsite. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.652(a)(1): Employees were not protected in an excavation from a cave-in by an adequate protective system designed in accordance with paragraph (b) and (c) of this section. On or about August 17, 2016, at the above addressed jobsite, employees were performing work on a residential sewer line in a trench that measured approximately 5 to 7 feet in depth. The trench was not protected with cave in protection, thereby exposing the employees to cave in hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
