29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures:  On March 26, 2016, the employer did not ensure that specific procedures were developed for locking-out a gasket loading beller machine to protect employees from the accidental release of stored energy exposing employees to struck-by and caught-in hazards on line one of the facility.
