29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard was not attached to the boom or basket when working from an aerial lift:    Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).      a) Job site, 1813 State Route 819, Greensburg, PA: On or about August 23, 2016, during construction activities, the employer did not ensure that employees used fall protection systems/devices which were attached to the aerial lift boom or basket as they used it while they worked.

29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems:      a) Job site, 1813 State Route 819, Greensburg, PA: On or about August 23, 2016, during steel erection activities, the employer did not ensure that employees used fall protection systems/devices where exposed to fall hazards of up to approximately 18 to 20 feet.
