29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a) On August 13, 2016 employees who clean up the Mill was not trained as a authorized lockout employee in that during cleaning up of the Mill , Shaker Table and Chipper was not turned off and locked out.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:   a) Employees who perform servicing and/or maintenance on or around the Mill Line were not required to: (1) Physically locate and operate the electrical disconnect in such a manner as to isolate the machine or equipment from the energy source in that on August 13, 2016 the mill was not shut down during clean up. (2) Affix a lockout or tagout devices to the electrical disconnect.

29 CFR 1910.219(e)(3)(i): Vertical and inclined belts shall be enclosed by a guard conforming to standards in paragraphs (m) and (o) of this section.   a) Machine guarding on the Wood Chipper machine main motor drive belts were inadequate,  in that the lower section of the guard did not enclose the moving belts exposing employees to pinch points.
