29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a) Employees were exposed to an unguarded point of operation while reaching into the Peddinghaus Peddieworker Ironworker to place and retrieve parts and perform other tasks during operation because the employees are not prevented from entering the point of operation by adequate guards or machine controls.    b) Employees were exposed to an unguarded point of operation while reaching into the angle shear to retrieve parts and perform other tasks during operation because the employees are not prevented from entering the point of operation by adequate guards or machine controls.    The John Leithoff was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii): , which was contained in OSHA inspection number 561518, citation number 1, item number 5 and was affirmed as a final order on October 30,2012, with respect to a workplace located at 609 N. Fulton in Payson, IL.
