Darrell Schmidt worksite located at 7860 Iron Horse, Monclova, Ohio: On or about August 24, 2016, the employer failed to ensure employees conducting residential construction activities were trained to recognize and avoid common hazards including but not limited to fall protection, ladder use, and personal protective equipment.

A. Darrell Schmidt worksite located at 7860 Iron horse, Monclova, Ohio: On or about August 24, 2016, the employer did not ensure employees were provided with proper eye protection while operating pneumatic nail guns to secure asphalt shingles. Five employees were exposed to potential flying object hazards while operating the pneumatic fasteners.

A. Darrell Schmidt worksite located at 7860 Iron Horse, Monclova, Ohio: On or about August 24, 2016, the employer did not ensure employees were protected from falls while installing weatherproofing on the two story residential structure.  The employees were exposed to a fall distance of approximately 18.00 feet to the lowest level from the 12/12 pitch roof.

A. Darrell Schmidt worksite located at 7860 Iron Horse, Monclova, Ohio: On or about August 24, 2016, the employer did not ensure the side rails of the aluminum extension ladder was extended at least three feet above the landing surface. Employees were exposed to falls while using the ladder to access the roof of the two story residential structure.
