29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:   NuTech Refinishing, Inc. at 1371 Willow Run Rd Greensboro, GA, the employer allowed an employee to be elevated on a work platform/man-basket that was attached to the forks of a Hyster forklift without first obtaining approval from the forklift manufactures or ensuring that the man-basket and fork attachment were designed by a certified professional engineer, on or about 3/30/16.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  Greensboro, GA -  Production Area - An employees was required to operate the Hyster propane powered forklifts and had not completed training, certification, and/or evaluation as specified by this standard, on or about 03/30/16.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
