29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: WB Contracting, Inc. - Intersection of Forkner Drive and Glendale Avenue, Decatur, GA - The employer failed to protect employees working in a trench/excavation from potential danger of head injury from impact of falling objects by allowing them to work in an unprotected trench approximately 6'1- 6'2 deep installing a water pipe/line. Employees were exposed to overhead hazards, on or about August 25, 2016. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: WB Contracting, Inc. - Intersection of Forkner Drive and Glendale Avenue, Decatur, GA - The employer failed to protect employees working in a trench/excavation from cave-in hazards by allowing them to work in an unprotected trench approximately 6'1- 6'2 deep with inadequate means of egress, on or about August 25, 2016. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
