29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   WB Contracting, Inc. - Intersection of Forkner Drive and Glendale Avenue, Decatur, GA - The employer failed to protect employees working in a trench/excavation from potential danger of head injury from impact of falling objects by allowing them to work in an unprotected trench approximately 6'1- 6'2 deep installing a water pipe/line.  Employees were exposed to overhead hazards, on or about  August 25, 2016.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  WB Contracting, Inc. - Intersection of Forkner Drive and Glendale Avenue, Decatur, GA - The employer failed to protect employees working in a trench/excavation from cave-in hazards  by allowing them to work in an unprotected trench approximately 6'1- 6'2 deep with inadequate means of egress, on or about  August 25, 2016.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift:   WB Contracting, Inc. - Intersection of Forkner Drive and Glendale Avenue, Decatur, GA - The employer failed to ensure that trenches and excavations are inspected by a competent person which led to employees being exposed to cave-in hazards while working in a trench/excavation approximately 6'1"-6'2" deep without cave-in protection, on or about  August 25, 2016.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c):  WB Contracting, Inc. - Intersection of Forkner Drive and Glendale Avenue, Decatur, GA - The employer failed to protect employees working in a trench and not in stable rock from a sidewall cave-in hazard. In that, employees were allowed to work in a trench with 6'1"-6'2" deep vertical sidewall that was not protected by adequate sloping/benching or a wall support system, on or about  August 25, 2016.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
