29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) The employer did not ensure that the Summit Model 20X120B metal lathe located in the repair shop was guarded. Employees were exposed to a struck-by hazard. (b) The employer did not ensure that the unused portion of the Kalamazoo Model H9AW horizontal metal cutting band saw located in the repair shop was guarded. Employees were exposed to an amputation and/or struck-by hazard. (c) The employer did not ensure that the two Pedarazoli Model Brown 250 cold metal circular saws located in the shops were guarded. Employees were exposed to an amputation and/or struck-by hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to the joints or terminal screws:  At the 4545 Harry St facility, the employer did not ensure that the strain relief on the male plugs providing electrical power to the two Parker Hannifin par-flange machines would prevent any pull from being directly transmitted to the joints or terminal screws within the plug.
