29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed: a) 400 Stamford Dr., Rogers Walk, Mount Laurel, NJ: On or about 08/27/2016 an employee taping drywall seams was using a section of rolling scaffold ladder frame as a portable ladder. The ladder frame was not designed for that purpose. The frame slipped and resulted in the worker falling approximately 12 feet down an elevator shaft.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement. a) 400 Stamford Dr., Rogers Walk, Mount Laurel, NJ: On or about 08/27/2016 an employee taping drywall seams was using a section of rolling scaffold ladder frame as a portable ladder. The ladder frame used on the second floor landing adjacent to the elevator shaft had not been secured from accidental displacement. The frame slipped and resulted in the worker falling approximately 12 feet down an elevator shaft.
