29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    a) A reach-type forklift was used by the employer to unseat cross-members within a steel storage rack, where operator(s) had not received safety training & certified competency evaluation for the type of powered industrial truck prior to assigned use.    Condition(s) existed on and before 8/27/2017.

29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled:    a) A reach-type forklift was used by the employer to unseat cross-member(s) from and unanchored, partially dismantled steel storage rack.  Lifting of the inter-connected cross-member(s) in the rack system caused instability, exposing employee(s) to struck-by hazards from collapsing rack components.    Condition(s) were noted on or about 8/27/2016.
