Location: 11 Ellsworth Ave. Cambridge, MA    On or about 8/26/16, an aluma-pole pump jack scaffold platform not secured overhung past the upright more than 3-feet exposing an employee to fall hazards of greater than 9-feet, 8-inches.

29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement:    Location: 11 Ellsworth Ave. Cambridge, MA    On or about 8/26/16, an aluma-pole pump jack scaffold was unstable and not level due to one of the aluma-poles that was set up over a hole in the ground exposing an employee to fall hazards greater than 9-feet, 4-inches.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.  Location: 11 Ellsworth Ave. Cambridge, MA  On or about 8/26/16, an employee was exposed to fall hazards of greater than 9-feet, 8-inches while working from an aluma-pole pump jack scaffold due to the poles not being secured to the structure with a tie back device at the bottom of the scaffold.

Location: 11 Ellsworth Ave. Cambridge, MA    On or about 8/26/16, the aluma-pole pump jack scaffold platform an employee was working from was damaged with a broken rung and the platform had been previously repaired with rivets.
