29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    (a) Location - 375 Turnpike Street, North Andover, MA:  On or about 8/30/16, employees were exposed to electrical shock and burns when installing cable equipment within 10 feet of a 120/240 secondary overhead power line.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:    (a) Location 375 Turnpike Street, North Andover, MA:  On or about 8/30/16, employees were exposed to falls when working in an aerial lift with no fall protection installing an antenna.
