29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):     On or about August 22, 2016, the employer failed to ensure employees operating Press #53, a Minster #5, Model number 5, serial number 5-9028, 45 ton open back inclinable press with a three (3) inch stroke were protected against contact with the point of operations. The two hand control was bypassed allowing employees access to the point of operation.
