29 CFR 1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:  On or about July 28, 2016:  a) An employee suffered lacerations to the left hand and suffered a partial amputation of the left pinky finger when  the employer failed to properly guard the circular handfed ripsaw in use by the employee.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.   On or about July 28, 2106:  a) The employer failed to report to the local OSHA office a in-patient hospitalization that occurred as a result of an amputation to an employees pinky finger, while using an unguarded table saw.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
