29 CFR 1926.200(g)(2): The employer did not ensure that all traffic control signs or devices used for protection of construction workers conformed to Part VI of the Manual of Traffic Control Devices (MUTCD) Millennium Edition, December 2000, 1 FHWA, which are incorporated by reference:    (a) Shaw Construction, LLC @ 1615 Pennsylvania Street, Denver, CO 80223:  On or before July 15, 2016, as part of injury/accident prevention responsibilities under its safety program, the controlling and exposing employer did not ensure that the required traffic control signs and devices were allocated so that road users approaching the street intersections from all directions were advised and warned about the hazards they were approaching and had sufficient advanced warning.  This missing signage includes, but is not limited to, Construction Ahead,  Construction Moving Equipment,  Entering/ Leaving Jobsite, Merge to the Left, Merge to the Right, and Flaggers Ahead, to address work zone hazards including concrete trucks entering/departing the construction site on the north and east sides of the street gate entrance.  This condition exposed construction workers to the hazard of being stuck-by approaching vehicular traffic.  On July 15, 2016, a flagger working at the intersection of East 16th Avenue and North Pennsylvania Street was struck-by a concrete truck as the truck was leaving the construction site.  The employee suffered abdominal injuries.      Abatement Note:  Part VI of the Manual of Uniform Traffic Control Devices (MUTCD) Millennium Edition, December 2000, 6B.01 provides that the control of road users through a temporary traffic control zone shall be an essential part of highway construction, utility work, maintenance operations, and incident management.  Sections 6C.03 and 6C.04 discuss requirements associated with advance warning areas, providing that the advanced warning area is that section of the highway where road users are informed about the upcoming work zone or incident area.
