The procedures were not clear and specific, in that, they did not include the isolation of the hydraulic energy and blocking of the ram at the #5857 Farquar press and the isolation of the steam energy and blocking of the ram at the #2061 Blacksmith hammer during die changes.

Deficiencies such as, but not limited to, air cooler fan motor lock out, inaccurate voltage, and hydraulic jack use as blocking between the rams and anvil bases during the changing of dies as a maintenance service task.
