The elevated oiler platform for the #2061 Blacksmith 5,000 pound steam hammer had an unprotected ladderway opening.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:    On or about September 1, 2016, the employer did not ensure energy control procedures were clear and specific for shutting down and blocking the #5857 Faquar 1500 ton hydraulic press and the #2061 Blacksmith 6,000 pound steam hammer. The procedures were not clear and specific, in that, they did not include the isolation of the hydraulic energy and blocking of the ram at the #5857 Farquar press and the isolation of the steam energy and blocking of the ram at the #2061 Blacksmith hammer during die changes.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    On or about September 1, 2016, employees were exposed to a live electrical parts hazard. There was an industrial fan that provided air flow for cooling and air circulation at the #2222 Denison 6,000 pound trim press. The industrial fan had an on/off control switch that was damaged and open with the electrical contacts and wiring exposed.

Deficiencies such as, but not limited to, air cooler fan motor lock out, inaccurate voltage, and hydraulic jack use as blocking between the rams and anvil bases during the changing of dies as a maintenance service task. 29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:     On or about September 1, 2016, employees were exposed to unprotected electrical wiring at the #2222 Denison 6,000 pound  trim press. At the bottom of the main disconnect switch for the press the flexible conduit had a rigid elbow fitting that was missing the protective cover plate.
