On or about August 22, 2016 the employer did not ensure that employees lifting/transferring an intermodal container form a truck chassis to the ground using a wrecker at 3030 Warrenville Rd., Lisle IL 60532, were protected from falling from heights. Employees were using the under reach on the wrecker to attach the slings to the passengers side top posts on the intermodal container. employees were elevated approximately 8 feet above the ground. Employees were exposed to falls from heights. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Employees were lifting/transferring an intermodal container from the chassis of a truck to the ground using a wrecker. An assessment of the hazards and risks associated with the required task would have revealed the necessary equipment and protection required to prevent injuries at this site. Employees were not utilizing protective equipment such as but not limited to steel toes and hard hats. Employees were exposed to crushing hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
