29 CFR 1926.850(a): An engineering survey was not conducted and a written demolition plan was not implemented by a competent person to determine the condition of the framing, floors, and walls and possibly unplanned collapse of any portion of the structure:     a)  604 Cornell Street, Perth Amboy, NJ 08861. The employer allowed his employees to work in a two story house that was previously damaged by fire when a survey had not been conducted by a competent person to determine the condition of the framing, floors and walls and possibly of unplanned collapse of any portion of the structure exposing the employee to hazards associated with demolition work.      Incident occurred on or about 09/06/2016.

The employer allows employees to work in areas with unprotected wall openings that were not protected by a guarding system at approximately 42 inches. Thereby exposing the employees to fall hazards of up to 7 feet.     Occurred on or about 09/06/2016.

The employer allows employees to use stairways having four or more risers to access the second floor when the stairway was not equipped with one handrail and one stairrail system along the unprotected side or edge exposing the employees to fall hazards.
