29 CFR 1910.333(a)(2): Where exposed live parts were not deenergized, other safety related work practices were not used to protect employees who could be exposed to the electrical hazards involved:   a.  On or about September 3, 2016, circuit breakers that provided power to the LED LDR Troffer Retrofit Bracket Kit were not de-energized and other safety related work practices were not used to protect employees who could be exposed to the electrical hazards involved, an employee was hospitalized after he received an electrical shock while working on a live circuit.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a.  On or about September 3, 2016, an employee installing a 32 watt SYLVANIA lamp, was not protected from a 12 foot fall hazard by the use of a safety net system, personal fall arrest system, or any alternative fall protection.
