29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.     (a)New Generation Roofing at 6320 S Avalon Ave. Sioux Falls SD, 57108:  On or about September 2 2016, the exposing employer did not ensure that fall protection systems were utilized by employees engaged in re-roofing activities while working at the unprotected edges of a roof on a multi-family residential housing complex. Employees were exposed to a fall hazard of approximately 28 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     (a)New Generation Roofing at 6320 S. Avalon Ave, Sioux Falls SD, 57108: On or about September 2, 2106, the exposing employer did not ensure that each employee engaged in roofing operations were trained to recognize the hazards of working at elevation and trained in the use of fall protection. Employees engaged in roofing activities worked near the edge of a multi-story roof on a residential home without the use of a fall protection system. This condition exposed employees to a fall hazard of approximately 28 feet.
