Avalon Ave, Sioux Falls SD, 57108: On or about September 2, 2106, the exposing employer did not ensure that each employee engaged in roofing operations were trained to recognize the hazards of working at elevation and trained in the use of fall protection. Employees engaged in roofing activities worked near the edge of a multi-story roof on a residential home without the use of a fall protection system. This condition exposed employees to a fall hazard of approximately 28 feet.
