29 CFR 1926.150(c)(1)(i): Travel distance from any point of the protected area to the nearest fire extinguisher exceeded 100 feet:  415 SILAS DEANE HIGHWAY, WETHERSFIELD, CT:  On or about September 3, 2016, employees were installing an EPDM roofing system.  The installation of such a system involves the use of flammable adhesives.  While such adhesives were stored, there was no fire extinguisher accessible on the roof or within 100 feet.

29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the jobsite:  415 SILAS DEANE HIGHWAY, WETHERSFIELD, CT:  On or about September 3, 2016, employees were installing a new EPDM roofing system using flammable adhesives.  There was no fire extinguisher available on the roof.  There were two 3-gallon containers of Firestone Quick Prime Plus Primer (FP 24.8 deg F), three 5-gallon pails of Firestone Single Ply LVOC Bonding Adhesive (FP 14 deg F), and two 5-gallon pails of EPDM Bonding Adhesive BA-2004 (FP -0.4 deg F).

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  415 SILAS DEANE HIGHWAY, WETHERSFIELD, CT:  On or about September 3, 2016, employees who were installing a new EPDM roofing system were exposed to fall hazards when the hatch for accessing the roof was left open without guardrails or other means of fall protection.
