29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  A.) Primary Health Network, 30 Pinnacle Drive, Clarion, PA  16214, on or about September 6, 2016 and times prior thereto:  A key fob and badge system was utilized to open the exit access doors that lead into the stairwells.  The system locked the exit access door from the inside, restricting access to the stairwell area.  The system prohibited employees from utilizing the exit access door if their key fob badge was not immediately available, and would restrict access to the exit discharge door within the stairwell area.  Other means to manually disable the exit access door locking mechanism were not available.

Primary Health Network, 30 Pinnacle Drive, Clarion, PA  16214, on or about September 6, 2016 and times prior thereto:  Fire extinguishers including, but not limited to, the Badger Dry Chemical Fire Extinguisher (M/N:  B10M, S/N:  AP-852230) were last inspected on April 12, 2016.
