29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  Note: 29 CFR 1926.602(d) Powered industrial truck operator training. The requirements applicable to construction work under this paragraph are identical to those set forth at 1910.178(l) of this chapter.  On or about, September 7, 2016, the employer did not ensure the employees successfully completed training and had their performance evaluated prior to operating a powered industrial truck.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:  On or about September 7, 2016, employees were exposed to a fall hazard in the concrete stairwell on the North side of the building and the stairway was not equipped with a handrail.
