29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about September 08, 2016, on the roof of the Signature Hotel where employees were exposed to a fall hazard greater than 6 feet while working on the roof without a fall protection system.

29 CFR 1926.1052(c)(4): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structural members were not provided between the top rail of the stairrail system and the stairway steps:    On or about September 08, 2016 at the Signature Hotel site, where employees were exposed to a fall hazard when using the stairway that did not have a midrail, mess or equivalent.
