29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  On or about September 8, 2016, and times prior thereto, at a residential structure, where employees were performing framing activities, debris including but not limited to boards with nails protruding and pieces of lumber were scattered about the construction site, a condition that exposed employees to the hazards of tripping, slipping and falling, as well as to puncture hazards.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.:   On or about September 8, 2016, and times prior thereto, at the jobsite, employees were stick framing roof trusses using Hitachi pneumatic nail guns without the use of eye protection, exposed to hazards of flying projectiles.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about September 8, 2016, and times prior thereto, on a residential framing, employee(s) were observed stick framing roof trusses on a 3,500 square foot, one story residence at heights greater than 6 feet without the use of fall protection systems, exposing employees to fall hazards.
