29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: On or about September 8, 2016, and times prior thereto, at a residential structure, where employees were performing framing activities, debris including but not limited to boards with nails protruding and pieces of lumber were scattered about the construction site, a condition that exposed employees to the hazards of tripping, slipping and falling, as well as to puncture hazards.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.: On or about September 8, 2016, and times prior thereto, at the jobsite, employees were stick framing roof trusses using Hitachi pneumatic nail guns without the use of eye protection, exposed to hazards of flying projectiles.
