29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe-boards, safety net systems, or personal fall arrest systems:  a) On or about 09/12/2016 - 3702 Mays Bend Road, Pell City, AL, employees removing existing asphalt shingles from a steep-sloped (12:12) roofing system at approximately 13 feet above the ground were unprotected from falls.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about 09/12/2016 - 3072 Mays Bend Road, Pell City, AL, employees were using a 24-foot Werner fiberglass extension ladder to access the roofing surface that was not extended 3 feet or more above the landing surface.
