29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.      a) Building Exterior. An employee was exposed to a fall hazards of approximately 10 feet while performing the installation of  two gas tanks without the use of a personal fall arrest system or other feasible means of fall protection, on or about 9/9/2016.

Excavated soil removed from a thirteen (13) foot deep excavation was not placed more than 2 feet from the edge of excavation, on or about 9/9/2016.

The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i).    a) Building Exterior. An employee was performing the installation of gas tanks inside an excavation approximately 13 feet in depth that was not protected by an adequate protective system, on or about 9/9/2016.
