29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury.    On 5/27/2015, employees from JJJ ROOFING were installing roofing materials on a roof of a located at 2935 S. 29th Street, Lincoln, NE 68502.  The workers were operating pneumatic nail guns without any form of eye or face protection equipment.  The workers were exposed to a struck-by hazard.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    On 9/12/2016, The owner and his employees from JJJ ROOFING were installing roofing materials on a roof of a located at 2935 S. 29th Street, Lincoln, NE 68502.  They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems.   The owner and four employees were observed on the roof of the building without fall protection, exposing them to a fall to the ground below.
