OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards when standing and working on platform.       (a) Location - 582 Westford Street, Lowell, MA:  On or about 9/13/16, employees were exposed to falls and possible collapse while using a material hoist as work platform.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a) Location 582 Westford Street, Lowell, MA:  On or about 9/13/16, employees were exposed to falls working on a roof greater than 20 feet from the ground.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) Location - 582 Westford Street, Lowell, MA:  On or about 9/13/16, employees were exposed to falls when accessing ladders that were not three foot above landing.
