29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a)  822 N. 19th Street, Allentown - On or about September 14, 2016, employees are performing roofing work, up to 28 feet above grade, without utilizing fall protection.    Whitaker Roofing and Siding, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 996230, citation number 1, item number 1 and was affirmed as a final order on 10/2/14, with respect to a workplace located at 3731 Broadway Street, Bethlehem, PA.    Whitaker Roofing and Siding, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1065391, citation number 1, item number 1 and was affirmed as a final order on 10/191/15, with respect to a workplace located at 229 S. 15th Street, Emmaus, PA.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:            a)  822 N. 19th Street, Allentown -  On or about September 14, 2016, the ladders used to access the upper level did not extend at least three feet above the upper landing surface.    Whitaker Roofing and Siding, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 996230, citation number 1, item number 2 and was affirmed as a final order on 10/2/14, with respect to a workplace located at 3731 Broadway St., Allentown, PA.
