29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems:  The employer is failing to protect employees from falls while working on a roof at 3331 Jamestown Road, Lincoln, NE, 68516. Employees of Nery Rivera were observed exposed to fall hazards exceeding 9 feet while installing weatherproofing materials on a residential structure.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  The employer is failing to protect employees from fall hazards due to the ladder used for access and egress not extending at least 3 feet above the upper landing surface of the roof. This was most recently documented at 3331 Jamestown Rd, Lincoln, NE, 68516.  Employees of Nery Rivera were observed utilizing an extension ladder that did not extend at least 3 feet above the upper landing surface.
