On or about September 13, 2016, at a construction site when an employee was exposed to a fall hazard while attaching wood to the frame of a building without the scaffold being fully planked.

29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph. On or about September 13, 2016, on a construction site, an employee was exposed to fall hazard when while working from a 6 foot A-Frame ladder placed on top of a scaffold to increase the working level height of the employee.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. On or about September 13, 2016, at a construction site when an employee was exposed to a fall hazard while climbing the frame of the scaffold to access the scaffold platform more than 2 feet above a point of access.
