29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).      On or about September 13, 2016, at a construction site when an employee was exposed to a fall hazard while attaching wood to the frame of a building without the scaffold being fully planked.

29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph.    On or about September 13, 2016, on a construction site, an employee was exposed to fall hazard when while working from a  6 foot A-Frame ladder placed on top of a scaffold to increase the working level height of the employee.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    On or about September 13, 2016, at a construction site when an employee was exposed to a fall hazard while climbing the frame of the scaffold to access the scaffold platform more than 2 feet above a point of access.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall protection.       On or about September 13, 2016, on the eastside of a roof, employees were exposed to fall hazards of more than 6 feet while engaged in framing activities near the unprotected sides and edges of a working surface without a fall protection system.
