29 CFR 1910.151(c):Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use. On or about 14 September 2016, the eyewash station in the production area was located more than 10 feet away from the forklift battery charging station; exposing employees to hazards of corrosion, severe irritation or permanent damage to the eyes.

29 CFR 1910.212(a)(2): Guard(s) on the horizontal saw were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: On or about 14 September 2016, in the production area, employees were utilizing the horizontal bandsaw (Slitter) to cut foam without guards in place to protect the employees from the unused portion of the blade, exposing employees to the hazard of being struck by the revolving blade.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about 14 September 2016, a wall fan which had been mounted on a pole adjacent to a vertical saw in the production area had the outer sheathing pulled away from the cord plug, exposing employees to electrical hazards of 120volts.
