29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about September 8, 2016 and at times prior thereto, an employee who was standing in the vicinity of an employee who was performing residential framing on an elevated work platform without wearing head protection, exposed to the hazard of having boards or tools strike him in the head.

29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about September 8, 2016 and at times prior thereto, an employee who was standing on an elevated work platform was engaged in framing a single story home without eye protection while using a hammer and a pneumatic nail gun. This condition exposed the employee to hazards to the eye.

29 CFR 1926.403(b)(1)(i): Safety of equipment was not determined on the basis of suitability of the equipment for an identified purpose as evidenced by listing, labeling, or certification for that identified purpose.    On or about September 8, 2016 and at times prior thereto, at the jobsite located at or near 815 E Asher Drive, Rogers, Arkansas, Employees were using an outlet box to power their electrical equipment such as circular saws that was not labeled for use at a construction site.  This condition exposed employees to electrical hazards.

This condition exposed employees to electrical hazards.
