29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents: On or about September 15, 2016, at the worksite located at Via Lucca Street, Lot 10, in Tontitown, Arkansas, employees using pneumatic nails guns to perform roof framing tasks did not wear eye protection. This condition exposed employees to the hazard of eye injuries.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about September 15, 2016, at the worksite located at Via Lucca Street, Lot 10, in Tontitown, Arkansas, the Werner portable extension ladder used to access and descend the roof level did not extend 3 feet above the landing surface. This condition exposed employees to falls from elevations.
