29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about September 15, 2016 and at times prior thereto, employees were using pneumatic nail guns and hand tools such as hammers without eye protection while performing residential roofing construction. This condition exposed employees to struck by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about September 15, 2016 and at times prior thereto, employees were engaged in residential roofing construction approximately 14 feet above the ground below without fall protection. This condition exposed employees to fall hazards.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. On or about September 15, 2016 and at times prior thereto, employees were routinely exposed to fall hazards while employed as roofers without having training in fall hazard awareness. This condition exposed employees to fall hazards while they were performing residential roofing construction approximately 14 feet above the ground below.
