29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:    (a) In the outside staging area at the facility at 20 North main Street, Grafton, MA 01536, on 9/16/2016 stacks of pallets with "super-sacks" of abrasive grit were blocked so that when internal pallets of super-sacks were compromised the block became unstable and collapsed.
