29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:    a)    On or about September 15, 2016, and times prior, employees were exposed to injury while working in a trench in excess of 5 feet deep.  The shielding systems (trench boxes) were not used in an appropriate manner that would prevent case-ins due to excessive areas of exposed trench walls above the tops of the shields.  The shields were also installed with excessive space between the trench walls and the walls of the shields which would not prevent lateral movement of the shielding system in the event of a cave-in.
