OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to the hazard of being struck by heavy stone slabs or death: ESTABLISHMENT: The employer did not ensure through visual inspection, specific instructions and other appropriate means that the marble stone slabs placed on A-frame cradles which were inside a delivery truck were properly secured to prevent from displacement prior to performing unloading tasks. On or about 09/02/2016, while one of two (2) A-frame cradles was being pulled out from the delivery truck, two (2) marble stone slabs placed on the A-frame cradle were displaced and fell on the employee who was standing in between two (2) A-frame cradles. The employee sustained very serious injuries from the incident.
