29 CFR 1910.219(d)(1): Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.    WORKSHOP/PRESS DEPARTMENT:  The inner/outer and the adjustment areas of the flywheels of the following full revolution mechanical power presses were unguarded:    1.   #4 Minster 32 ton; inner smooth flywheel unguarded. ABATED DURING THE INSPECTION.     2.   #8 Federal 32 tons; adjustment area on the flywheel unguarded.        3.   #13 Niagara; outer hub on the flywheel  unguarded.        4.   #3 VO Super 15 tons; inner smooth flywheel/adjustment area unguarded.  ABATED DURING THE INSPECTION.

WORKSHOP/PRESS DEPARTMENT:  The flexible cord, installed for the #5 Niagara mechanical power press was being used to power #2 Minster mechanical power press, and it was hanging between two (2) power presses.

29 CFR 1910.133(a)(3): The employer shall ensure that each affected employee who wears prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design, or wears eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses.  WORKSHOP:  The employer had not ensured that each employee wearing prescription lenses while working in the production/tool department was wearing appropriate eye protection.

29 CFR 1910.134(k): Training and information. This paragraph requires the employer to provide effective training to employees who are required to use respirators. The training must be comprehensive, understandable, and recur annually, and more often if necessary. This paragraph also requires the employer to provide the basic information on respirators in Appendix D of this section to employees who wear respirators when not required by this section or by the employer to do so.  WORKSHOP:  The employees, optionally wearing filtering type dust masks while performing cleaning task were not given the information contained in the Appendix D of the respiratory protection standard.
