29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):   a) On or about September 20, 2016, and at times prior, the employer did not ensure through regular and frequent inspections of the site, materials, and equipment, that adequate fall protection was in place for 5 workers on a residential roof greater than 10 feet high.
