29 CFR 1926.451(f)(1): Scaffold and/or scaffold components were loaded in excess of their maximum intended loads or rated capacities, whichever was less:    On or about October 24, 2016, at the above addressed jobsite, the 5/8 eye bolts used to support the swing stage scaffold had their rated capacities exceeded, causing the eye bolts to fail and the scaffold to fall approximately 40 feet to the ground below.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:    On or about October 24, 2016, at the above addressed jobsite, the 5/8 eye bolts used to support the swing stage scaffold were not screwed fully into the drop-in anchors and the drop-in anchors were installed below the roof surface.
